CHRONIC DISEASE SELF-MANAGEMENT EDUCATION

These six week workshops is designed to empower adults to better manage their chronic conditions such as diabetes, arthritis, chronic pain and heart disease with the goal of giving individuals confidence and skills to allow them to actively manage their health care and reduce unnecessary visits to the Emergency Department or the physician(s).

Chronic Disease Self-Management Program (CDSMP) focuses on solutions to problems, caused by living with ongoing health conditions such as pain, fatigue, sleep, managing medications and difficult emotions.

Diabetes Self-Management Program (DSMP) includes diabetes specific information such as meal planning, foot care, delaying complications, dealing with stress and planning for sick days.

 

A MATTER OF BALANCE

A Matter of Balance (MOB) addresses the fear of falling that many older adults experience.  People who develop this fear often limit their activities, which can result in physical weakness, making the risk of falling even greater. This program is designed to reduce the fear of falling and increase activity levels among older adults. 

FALLSTALK

FallsTalk is an individual program for anyone who has experienced a fall or regular loss of balance; regardless of walking ability, medical condition, mobility or fitness level. The program begins with a personal FallsTalk Interview in-home or community space to discuss their unique situation. The intervention consists of initial and follow-up interviews with a trained facilitator, daily personal reflection (2-3 min.), 3 brief weekly and then monthly check-in calls. Clinical trials and community results provide evidence that FallsTalk significantly reduces falls compared to untreated fallers. Our FallsTalk program is made possible by a grant for the Greater Lynchburg Community Foundation.

HEALTHY IDEAS

Healthy IDEAS is a depression self-management program designed to detect and reduce the severity of depressive symptoms in older adults with chronic conditions and functional limitations. It includes screening and assessment, education, referral to appropriate health professionals, and behavioral activation.
